You do not need to read the entire SGB V. You do need these five numbers and four paragraphs. Anything an insurer or broker tells you about eligibility, contributions or employer share is built on top of them.

How much does English-service PKV cost in 2026?

Employers cover roughly half of a PKV premium for employees, capped under § 257 SGB V. The cap is calculated against the BBG, so the employer share is not unlimited. Check the cap before you assume your employer pays half of a 600 EUR premium in full.

German fine print to watch even on English-service contracts

English service does not change German contract law. These four items are the ones that quietly create problems for expats later.

Binding language of the policy

Some insurers send an English summary but keep the German Versicherungsbedingungen as the legally binding text. In a dispute, that is the document that counts.

Pre-existing conditions (§ 19 VVG)

PKV insurers must ask about your medical history. Answer in full, in writing, even if the question feels intrusive. An omission can void the contract years later.

Deductible (Selbstbeteiligung)

A higher Selbstbeteiligung lowers the monthly premium, but it applies per calendar year. Two GP visits in January can use up the whole budget.

Anwartschaft for international moves

Ask for an Anwartschaft option in writing if there is any chance you will leave Germany temporarily. Without it, you may face a fresh health check on return.
